Please see the information below following up on the release of the new Phases of Panel Management process last week (June 18th). We’ve included some additional resources to support you through this process. First off, the checklist sheet outlines the steps required for completion of each phase. We are working for these to be linked to COMET, but for now please use the excel version. In the same document you will find an outline of the EMR tasks involved in each phase. We see this as a basic guide to ensure that someone on the practice team knows how to use this functionality.

Additionally, here are some resources on calculating panel size, which is a requirement for Phase 1: Empanelment. There is a “right size calculator” worksheet created by Safety Net Medical Home, a Best Advice article by the College of Family Physicians, and a brief information sheet about what to do if a physician is overpaneled. These are all resources for you to use to engage in a conversation with physicians around “ideal” panel size and start thinking about supply and demand.
